Extended leave form
This form must be completed and returned to the School principal if you are taking leave for 3 days or more. Eg: family holiday. This does not include sick leave.
Individual Health Care Plan
Individual Health Care Plan to be completed by parent/carer. If your child is asthmatic the Asthma Care Plan can be obtained and completed by your GP and returned to school with the Individual Health Care Plan.
Request to administer prescribed medication
